I want to start keeping a daily journal of my pain, fatigue & anxiety levels, along with the amount of exercise I had each day and any other factors that may influence how I am feeling. I want to have this so I can show it to the new rheumatologist I am seeing Friday to give him a better idea of what I am experiencing and to show him the severity of my symptoms. I was diagnosed with fibromyalgia by another doctor, but want the second opinion of this rheumatologist on the diagnosis. I also cannot continue to live my life in this much pain. It is unbearable and I want to be able to convey that to this new doctor.

Have you ever kept a patient pain journal? If so, what tips can you give me on how to keep the most effective pain journal? What all should I include in my daily entries? All suggestions would be greatly appreciated. Thank you very much!
